Keefektifan Ketoprofen Dan Ketorolak Sebagai Analgesik Pada Pasien Pasca Bedah Cesar

Abstract

Pemberian analgetik yang efektif pada proses kelahiran cesar dapat meningkatkan mobilisasi ibu dan juga meningkatkan ikatan batin antara ibu dan bayi. Opioid sangat disukai oleh pasien, akan tetapi pemberian obat ini pada kelahiran cesar mengkhawatirkan karena dapat menyebabkan efek samping sedasi, mual, muntah, pruritus, dan konstipasi pada ibu (Benyamin et al., 2008) dan pada beberapa kasus pemberian opioid dapat menyebabkan depresi pernafasan pada bayi (Lam et al., 2012). Penggunaan anti inflamasi non steroid (AINS) dapat digunakan sebagai analgetik pilihan pada kelahiran cesar sehingga dapat  menghindari efek samping opiod. Ketoprofen dan ketorolak merupakan obat golongan AINS yang dapat digunakan untuk terapi analgetik pasca cesar.Penelitian merupakan penelitian analitik observasional. Pengambilan data dilakukan secara prospektif. Variabel yang diukur dalam penelitian ini adalah tingkat atau skala nyeri pada pasien pasca bedah cesar yang diukur menggunakan metode pengukuran skala nyeri Visual Analog Scale. Subyek penelitian sebanyak 24 pasien yang mendapatkan ketoprofen dan 26 pasien yang mendapatkan ketorolak. Data tidak terdistribusi normal, sehingga dianalisa dengan uji Mann-Whitney dengan taraf kepercayaan 95%.Nilai VAS pada pasien yang mendapatkan ketorolak lebih kecil daripada nilai VAS pada pasien yang mendapatkan ketoprofen. Akan tetapi nilai VAS pada jam ke-2 (p=0,907), jam ke-4 (p=0,303), jam ke-6 (p=0,089), jam ke-12 (p=0,449), dan jam ke-24 (p=0,593) berbeda tidak signifikan pada kedua kelompok.

Providing effective analgesics in the process of cesarean delivery can improve maternal mobilization and also increase the inner bond between mother and baby. Opioids are well-liked by patients, but the administration of these drugs in cesarean delivery is alarming because it can cause side effects of sedation, nausea, vomiting, pruritus, and constipation in the mother (Benjamin et al., 2008) and in some cases opioid administration can cause respiratory depression in infants (Lam et al., 2012). The use of non-steroidal anti-inflammatory drugs (NSAIDs) can be used as an analgesic of choice for cesarean delivery so that it can avoid opiod side effects. Ketoprofen and ketorolac are NSAIDs which can be used for post-cesarean analgesic therapy.Research is an observational analytic study. Data collection is carried out prospectively. The variables measured in this study were the level or scale of pain in patients after cesarean section which was measured using the Visual Analog Scale pain scale measurement method. The study subjects were 24 patients who received ketoprofen and 26 patients who received ketorolac. Data was not normally distributed, so it was analyzed by Mann-Whitney test with a confidence level of 95%.VAS values in patients who received ketorol ac were smaller than the VAS values in patients who received ketoprofen. However, the VAS value at the 2nd hour (p = 0.907), the 4th hour (p = 0.303), the 6th hour (p = 0.089), the 12th hour (p = 0.449), and the 24th hour (p = 0.593) was not significantly different in both groups.
